It is important to do an assessment for Zachary (the steps ADPIE.) While decoding the variables as stated in the readings, I would consider things like age, developmental vulnerability, and diagnosis/treatment. It is imperative to put much thought into the family support Zachary may or may not receive during hospitalization. We need to assess the family’s involvement and care they will be able to provide. As noted in the readings “the ability of family members to support in hospital, versus inability or effectiveness, can influence stress and coping!” (Thompson, 2009). Mobility is another important variable to keep in mind when assessing upon admission. Given that Zachary has dehydration, he will be hooked up to an IV for fluids. This may present a problem with play room socialization and should be noted when planning his Child Life services. We want to practice Family-Centered Care, meaning we want to include his parents in his care plan as heavily as possible. Zachary is young and perhaps afraid of his new admission to the hospital. His parents will know what he likes/dislikes and needs for comfort. His parents will be working 90 minutes away, so they will likely not be actively involved in bedside daily activities physically, but can be included in the planning process. As a CCLS, I would certainly designate Volunteers to play with Zachary. Likewise, I would come up with developmentally-appropriate activities at bedside for him and engage in Therapeutic Play when possible. Separation from parents will be a major stressor to this child. I would use the SMART format to write his goals. SMART is an acronym that stands for: Specific, Measurable, Achievable, Relevant, and Time-bound. I’m not sure of the child’s actual diagnosis, as this discussion just notes that he has dehydration (unsure if its dehydration only or dehydration as a result of a virus/ infection/ disease, etc that will keep him hospitalized longer) However, it should not take long to get re-hydrated and when considering “discharge” for Zachary, this “timebound” component should be thought through.
